EMS 458 - Adv.Trauma Care for Paramedic

This course provides an in-depth exploration of the assessment and management of traumatic injuries across all patient populations. Students examine trauma system organization, scene safety, and evidence-based approaches to life-threatening injuries. Emphasis is placed on rapid trauma assessment, airway and hemorrhage control, shock recognition, and management of head, chest, abdominal, musculoskeletal, and environmental trauma. Learners apply advanced clinical decision-making and critical thinking to perform field interventions, coordinate multidisciplinary trauma response, and ensure safe, efficient patient transport. Integration of ATLS, NAEMSP, and national trauma guidelines reinforces high-quality prehospital trauma care, documentation, and team-based leadership.

Competencies
  1. Demonstrate Trauma Systems & Scene Management
    1. Understand trauma system organization, triage criteria, and trauma center levels
    2. Perform structured scene size-up and apply PPE for safety in trauma settings
    3. Identify mechanisms of injury and determine need for specialized resources (e.g., air medical)
  2. Perform Primary Trauma Assessment
    1. Conduct rapid trauma surveys with focus on airway, breathing, circulation, disability (AVPU/GCS), and exposure
    2. Apply cervical spine precautions, prioritizing life threats during primary assessment
  3. Examine Airway and Ventilatory Management in Trauma
    1. Maintain airway patency while preserving spinal alignment
    2. Perform advanced airway techniques for trauma patients
    3. Recognize signs of airway compromise and ventilatory failure
  4. Demonstrate Hemorrhage and Shock Management
    1. Identify external and internal hemorrhage before applying bleeding control measures (tourniquets, hemostatics)
    2. Recognize types of shock in order to initiate appropriate fluid resuscitation or damage control resuscitation strategies
  5. Evaluate Chest and Abdominal Trauma
    1. Assess for life-threatening thoracic injuries (e.g., pneumothorax, flail chest) in order to perform indicated interventions (needle decompression)
    2. Identify signs of abdominal/pelvic trauma before initiating stabilization and rapid transport
  6. Evaluate Musculoskeletal and Soft Tissue Trauma
    1. Assess for fractures, dislocations, crush injuries, and soft tissue damage
    2. Apply appropriate immobilization/splinting techniques while monitoring for neurovascular compromise
  7. Assess Head, Facial, and Spinal Trauma
    1. Perform neurologic assessment using GCS, recognizing signs of increased intracranial pressure
    2. Manage facial injuries and protect the airway
    3. Apply spinal motion restriction when indicated
  8. Evaluate Burns and Environmental Trauma
    1. Classify burn severity using TBSA and depth estimations
    2. Initiate burn care and fluid management, recognizing inhalation injuries.
    3. Manage environmental injuries such as hypothermia, frostbite, and heat stroke
  9. Examine Trauma in Special Populations
    1. Modify assessment and treatment for pediatric, geriatric, obstetric, and special needs trauma patients
    2. Recognize age- and condition-specific considerations in trauma response and documentation
  10. Analyze Traumatic Cardiac Arrest
    1. Apply trauma-specific resuscitation algorithms
    2. Identify reversible causes for treatment (e.g., tension pneumothorax, hypovolemia)
  11. Display Effective Communication, Documentation and Handoff
    1. Accurately document trauma assessments, interventions, and responses
    2. Communicate using standardized formats (MIST, SBAR) during transitions of care
  12. Demonstrate Evidence-Based Practice and Quality Improvement
    1. Apply current trauma care guidelines (e.g., ATLS, NAEMSP)
    2. Participate in debriefings and QI processes to improve trauma care delivery
