ES4980 - Campus Health-Medical Records Procedure

- To establish procedures for obtaining and storing student health records, sharing health information with appropriate personnel.
- Procedure
- All current DMACC students seen in Campus Health will fill out paperwork in accordance with Campus Health protocols and procedures. Completed paperwork is kept in a locked file cabinet in a locked office for 2 years. If a current student has not visited Campus Health for 2 years, charts are removed from the filing system and scanned to a secure DMACC health library server. These scanned records are only accessible to Campus Health Specialist/Nurse Practitioner. Paper records are sent to a secure/locked location at DMACC- currently the basement of Bldg. #6, and then destroyed after 7 years. Release of Information is required for any sharing of student medical history between DMACC service areas or outside health agencies.
- Staff is encouraged to utilize their insurance and primary healthcare provider for health needs. Staff Blood Pressure can be done as a courtesy depending on availability of Campus Health Nurse. Faculty/Staff will be given their Blood Pressure reading on a B/P recording card. Faculty/Staff are encouraged to communicate all abnormal results to their primary physician for further evaluation/follow-up and questions. Staff Blood Pressures are not kept on file in Campus Health.
