BS5524 - Report on Personal Injury, Accident or Illness

1. Institutional Regulations
   
   Proper records shall be made of all reported personal injury accidents or illnesses and permanently filed for future reference.
    
2. Procedure
   1. As soon as practical after any reported personal injury accident or illness, a report shall be written including all pertinent information required on the DMACC Incident Report by the person to whom the accident or illness was reported. In the case of an employee, the form is to be completed by either the employee or the employee’s immediate supervisor.
      1. Forms are available from security personnel or the campus nurse.
      2. If injury results from “Physical Assault,” please see Procedure Number BS 5523.
