BS5512 - Work Requests - Emergency and Routine Maintenance

  1. Institutional Regulations
    1. Physical Plant shall routinely perform maintenance to assure continual operation of College equipment and buildings directly under the control of the Physical Plant.
    2. Work requests shall be required for the performance of maintenance and/or repair of equipment (except computers) under the control of DMACC Departments
    3. All maintenance requests shall be scheduled and performed on a priority basis.
  2. Procedure
    1. Emergency requests shall have the highest priority. Emergencies consist of, power outages, roof leaks, significant room temperature control problems, water leaks, gas odor, or other similar events which may adversely effect the learning and working environment, create safety problems and cause damage to the buildings. Such requests shall be telephoned to the Physical Plant for immediate attention.
    2. Non-emergency requests shall be handled on a priority basis and should be in writing, either by:
      1. Communicating the request work to the Physical Plant utilizing the preprinted Physical Plant work order,
      2. E-mailing the Physical Plant Secretary including:
        1. Date
        2. Requester name and phone number
        3. Index to charge expenses to
        4. Location of work to be performed
        5. Detailed description of work requested
        6. Critical deadline
        7. Copied to the appropriate budget manager for prior approval
      3. If an estimate is requested prior to performing the work, this should be indicated in the Email or on the work request.
    3. When appropriate, the cost of repair to equipment under the control of DMACC departments shall be charged back to the Department. This may include such items as food service machines, laboratory equipment, welding machines and audiovisual equipment. Physical Plant shall supply an estimate, if requested, before repairs are performed.
